Basically, if you show up and there's a problem [1] with your registration, this is the form you'll have filled out.
1: Mainly, if you are not yet registered, if there is an error or omission in your entry on the Permanent Register, and/or if you are dead [2]. In the last case, the form is traditionally requested by someone else on your behalf.
The Canadian part of Walking Dead has to have involved a lot of F0521s.
2: Yeah, it seems to me like 'I am dead' should be covered by error or omission but it is not.
