Definitely not diabetic. Very likely not kidney-related. The vet thinks he's a textbook example of hyperthyroidism. Treatable and not expensive.

Slight heart murmur but otherwise she said he is in good shape and looks much younger than 18.

Given the Groucho's age, James is letting his readers know it's not bad news. This is appreciated by some.

I suppose it would be wrong to send an eel to Groucho as a present, just so James would be caught in a paradox.

I think I was thinking vaguely of the infamous eel scene in The Tin Drum. Being eel-free would probably be good in that context as well.
